The process for requesting an accident report depends on where the accident occurred and which agency investigated it.   3. Enter information identifying the collision.
  4. Alternatively, complete SCDMV Form FR-50 and take it to a branch or mail it as instructed.

The current SCDMV fee for a mailed request is $10 per collision report.

What the Officer Handed You at the Scene Is Not the Report

The paper an officer gives you after an accident is generally the FR-10 insurance verification form, not the complete collision report. The full police report includes additional collision details and may be needed for an insurance claim.
